It's not off, i just nicked it with a pair of scissors when i was cutting some bacon

I can play for a couple of minutes but thne it starts to open up again. On of my friends sliced his thumb up really badly on an open tuna can and caused some nerve damage, he couldn't play for weeks! I'm guessing that pro guitarists avoid sharp objects

Super glue that shit together then get to work. My bass player had big blisters on his picking fingers so he tore off the skin and glued it back on. It worked for him but I'm not suggesting you do that.

Superglue does stick flesh amazingly well, there's a scene in Dog Soldiers when one of the guys says it was invented for patching up soldiers in Vietnam.. not sure how true that is though.
